Suzanne Kornblatt, LMSWSuzanne Kornblatt, LMSW

Suzanne Kornblatt received her graduate degree from Adelphi University and is a licensed social worker. She has been the Supervisor of SIBSPlace for South Nassau Communities Hospital since 2004. Ms. Kornblatt monitors overall operations of the SIBSPlace program including supervising the creation of the children's monthly therapeutic curriculum, planning and facilitating on-site and off-site special events and providing support and guidance to program participants. She serves as a liaison to families providing information and referrals and facilitates a weekly parent support group. In addition, Ms. Kornblatt offers staff and volunteers supervision and in-service training. She maintains the SIBSPlace website and creates quarterly newsletters to sustain public relations, increase community awareness and facilitate agency networking. Ms. Kornblatt co-facilitates community education trainings on siblings’ response to illness and anxiety management in children. She has lent her expertise to SNCH's World Trade Center Family Center by facilitating events, maintaining the website and providing administrative support. Ms. Kornblatt co-authored poster presentations for the International Society of Traumatic Stress Studies Conferences : 2005 in Toronto, Canada and 2006 in Los Angeles, California.

 

Uoanna Formont, LMHCJoanna Formont, LMHC

Joanna Formont is a licensed mental health counselor and has been the Coordinator of SIBSPlace for South Nassau Communities Hospital since 2004. Ms. Formont graduated from Teachers College, Columbia University with a degree in Counseling Psychology and has worked with children and families in crisis for over 10 years. Ms. Formont coordinates the development and implementation of an activities based curriculum for children's programming. She contributes to planning and co-facilitates on and off site activities and events and provides support and guidance to program participants. Ms. Formont maintains participant and volunteer records, completes intake, and manages monthly outreach. She assists Supervisor in format development of quarterly newsletter and with additional administrative tasks as needed. Ms. Formont assisted the World Trade Center Family Center by providing client and outreach services. She co-authored poster presentations for the International Society of Traumatic Stress Studies Conferences: 2005 in Toronto, Canada and 2006 in Los Angeles, California. Ms. Formont also co-facilitates community education trainings on siblings’ response to illness and anxiety management in children.

 

Kristin Dugan-Reilly, LCSW

Ms. Kristin Dugan-Reilly is a Licensed Clinical Social Worker. She received her Masters in Social Work in 1996 from Adelphi University. Ms. Dugan-Reilly joined South Nassau Communities Hospital staff in 2005 as a team member of the World Trade Center Family Center program. For over ten years she has worked with families who are in crisis. As Assistant Coordinator, Ms Dugan-Reilly works with the children and their families at SIBSPlace. She assists with the implementation and development of the SIBSPlace therapeutic activity-based curriculum.

Kerri L. Wagner, MA

Kerri L. Wagner, MA Creative Arts Therapies, comes to SIBSPlace with a diversified background in art therapy and Child-Life programs. Her professional experiences at Bellevue Hospital and Mount Sinai Medical Center in Manhattan prepared her for joining the SIBSPlace team. Ms. Wagner’s skills enhance the therapeutic curriculum developed by the SIBSPlace staff for their program participants.

 

Dani Forte

Dani Forte is an Assistant Counselor at SIBSPlace. Ms. Forte started with the program in 2004 as a high school volunteer and has been an employee of the hospital since 2006. Ms. Forte, a senior at Hofstra University, is an Early Childhood/Elementary Education and Psychology major. As Assistant Counselor, Ms. Forte assists the professional mental health staff in providing support to the children during programming, and in other organizational responsibilities.
